A classroom observation form to record early reading warning signs from Kindergarten to Grade 3 and decide next steps.

Before you start

This form records what you see in class. It is not a diagnosis. Diagnosis is made by a licensed professional, usually a psychologist or psychological associate. In Quebec, a speech-language pathologist (orthophoniste) can also diagnose. Rules differ by province.

The International Dyslexia Association notes that early oral language weaknesses often come before reading problems. It also notes that early help is especially effective. Use this form to act early.
